Mô tả: With the examiner palpating the abdomen below the right subcostal margin, the patient is asked to take a deep breath in and, on doing so, will be caught by sudden pain.

Cơ chế: On deep inspiration the lungs expand, pushing the liver downwards so the inflamed gallbladder is pushed onto the examiner’s hand, causing a sudden and unexpected sharp pain.

Ý nghĩa lâm sàng: While individual studies80–82 have shown sensitivity of 48–87%, specificity of 48–79% and PLR of 1.9 and NLR of 0.6 for Murphy’s sign, a systematic review83 showed a PLR of 2.8 but could not rule out chance (95% CI, 0.8–8.6).
