Định nghĩa: khi so sánh huyết áp đo được khi nằm và khi đứng sau đó (trong vòng 3 phút), nếu HA tâm thu giảm xuống 20 mmHg hoặc HA tâm trương giảm xuống 10 mmHg trở lên. Thường có kèm tăng nhẹ nhịp tim, đặc biệt trong trường hợp hạ HA tư thế do mất nước hoặc mất máu (nếu bệnh nhân đang dùng ức chế beta thì có thể không thấy phản ứng tăng nhịp tim này.
